A very heavy bug associated with all versions before 1.4.1 is fixed. This bug potentially could happen in low TPS servers and servers that use Slimefun, but now it got fixed thanks to the @TheWylot report. I recommend updating ASAP or your server is vulnerable to the dupe exploit.
