CS-API Updates

• All hooks and placeholder replacers can be enabled/disabled as you wish in a new file (will be auto-generated in the CS-API folder). This is to prevent potential incompatibles due to API changes, although it will continue to check that required methods and classes exist before attempting to establish the hook.
• Fixes an issue with SQLite for insert statements potentially not being committed to the database due to auto commit.
• Updated the placeholder matching so MVdW placeholders (ex. {servermotd}) should be accessible for all messages utilizing the CS-API replacer.
• Fixes an issue with players potentially seeing incorrect items in a menu if another player has the same menu open.
• Other minor fixes.
