Removed the ability to eat item from offhand (may add back later just properly implemented)

Fixed a null pointer exception when attempting to set the name of a undefined item.

Added a new sub commands:
/food effect add
/food effect remove
/food effect list

Also fixed the bug were you could get over 20 food?!
