This is very good plugin for nuclear weapon, but I want to add text of radiation level, can you do it?

Sure, you can configure an additional level. Here's the default config for reference: https://github.com/uprial/customnukes/blob/master/src/main/resources/config.yml. You may edit your local server config file and add something like this:

scenario:
- effect1
- effect2
- effect
(...)
effect3:
type: effect
parameters:
radius: XXXX
players-only: false
effects:
- YYYY
duration: ZZZZ
