Whenever you run a command that tells you about difficulties, i.e. /arrowrain manage start, you'll be given a list of difficulties

Nightmare mode is just like insane mode, except if you get hit by an arrow you get struck by lightning.

Now if you get killed by an arrow, it'll look like this in chat:

Added insane difficulty, 5 arrows per tick
