Some fixes and a new option (death messages)
- fixed error when updating from 4.0.3 without server restart
- added option to disable death message replacements
- fixed error when attempting to undisguise everyone from console
- fixed custom names (and show names option)
- improved update checking error messages
- removed unnecessary files
