An error was reported to me about the problem with executing custom commands after the player dies. A list of changes:

- placeholder %time% has been fixed (previously it also added "min" to the time unnecessarily)
- placeholder %reason% has been replaced - now, instead of the entire message, only the reason for death is returned, not the entire ban screen
