Death Commands -----
Allows Commands or Broadcast to be Run on Player Death and Respawn2.1 Update Fix
Added Features:
1. Added Death cause: Fall by Player
2. Added Version Information to DeathActions.yml
Fixed Issues:
1. (AllDeaths) Commands/Broadcasts not running on Respawn
2. Internal placeholders modified to use allowed characters (%%)
Other Changes:
1. Minor Code Improvements
2. Removed Herobrine
3. Reworded Plugin messages
